Note at the bottom of this page reads: "The deaths numbered 5, 6, 9, 10, 12, 16, 17 & 21 were treated by physicians outside of my district and at some distance away and some very important cases consequently did not **** but to go **** ***, the case No. 17 was a person who died very soon after arriving within this district but not treated by the physicians here. Dr. Chapman was called but a short time before she died, the husband lives somewhere in the State of Ohio."
